"

Duties Description Provide direct supervision to multidisciplinary team members assigned – duties to include completing annual performance evaluation and probation reports, approving travel and timesheet submissions, and assisting with onboarding. Provide onsite field supervision and training to multidisciplinary teams during surveys conducted throughout the region (5 boroughs of NYC). Assist with providing training to new and existing staff as needed in conjunction with the Regional Nurse Trainer. Participate as a Team Coordinator on surveys. Monitor the flow of survey and complaint packages, summaries, Statement of Deficiencies (SOD) submissions, Plan of Correction reviews, and Post-Survey revisits and assigning work as needed to ensure deadlines are met. Ensure completed work is submitted and communicate with support staff so all survey kits can be closed. Conduct QA activities such as SOD review and editing, survey package audits, and survey documentation audits. Provide feedback and guidance to multidisciplinary team members regarding changes and improvements needed to maintain and improve the quality of program output. Provide updates on QA activities to Program Manager quarterly and as needed. Provide program coverage off-hours on a rotating basis.

Additional Comments Contingent-Permanent or Provisional, full-time, competitive appointment. Travel is required, including travel to some areas not served by public transportation. This position involves travel which may include locations not served by public transportation. Candidates appointed provisionally will be required to take and pass the next scheduled Civil Service examination for this title, when it is given, and be immediately appointable from the resulting eligible list to maintain employment. Candidates must pass SMQT within first year of employment, as required by the Centers for Medicare and Medicaid Services to conduct surveys.
