Minimum Qualifications Candidate must be reachable for appointment on Current Eligible List for this title. OR
Transfer:
Candidates must have permanent competitive status in the Program Operations Specialist 2 title and be eligible for lateral transfer; OR be eligible to transfer via section 70.1 or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.
Provisional Appointment:
In the event that there is not a valid list, candidates who meet the following promotional qualification may be qualified for a provisional appointment.
To qualify, candidates must have one year of permanent competitive or 55-b/ 55-c service either as a:
A. Program Operations Specialist 1 OR
B. Mental Hygiene Program Evaluations Specialist 2, Resources and Reimbursement Program Developmental Disabilities Specialist 1, Standard Compliance Analyst, Standard Compliance Analyst 1 (Developmental Disabilities) or Utilization Review Coordinator.
Duties Description OPWDD's Money Follows the Person (MFP) Program facilitates individuals moving from institutional residential settings into qualifying community settings with person centered planning and follow-up to ensure each person's needs are met and their quality of life is improved. This position fulfills critical functions in the OPWDD Division of Service Delivery, Regional Offices with on-going responsibilities in the operation of the Money Follows the Person (MFP) program and the ICF Transaction Plan. The incumbent will report to the OPWDD MFP Project Director / ICF Conversion Coordinator.
Primary Duties and Responsibilities:
-Management and oversight of complex, federally required data collection, mining, reporting and data base development and management for elements of the MFP Demonstration and the ICF Transition plan;
-Prepare reports for the Department of Health (DOH) that include all data elements and information required for the MFP demonstration in a timely manner, schedule meetings with DOH to review and address any barriers of obstacles to transitioning individuals from institutional settings to community based settings;
-Provide updates to the MFP Director for the semi-annual CMS report, Operational Protocol, and Guidance Documents as needed;
-Set agenda for monthly conference calls with Regional MFP Leads and ICF Liaisons across New York State to identify and address current and future needs of individuals who reside in ICF's;
-Develops and implements record-keeping systems and controls for data management of the MFP program, making recommendations to the MFP Director as appropriate;
-Conducts research into specialized populations residing in Skilled Nursing Facilities to identify individuals who qualify for the MFP program;
-Maintain an update tracking mechanism for data that identifies clinical support needs and reporting activities for individuals currently residing in ICF's to access current and future needs regarding ICF capacity in New York State; analyze the data and prepare reports that will be used by Administrative staff in strategic planning for ICF strategy.
