Duties Description POSITION DESCRIPTION:
The Office for People With Developmental Disabilities (OPWDD) provides high-quality, person-centered services, supports and advocacy to approximately 136,000 individuals with developmental disabilities in New York State. We are guided in our interactions with the people we serve and each other by compassion, dignity, diversity, excellence, and honesty.

The Office for People With Developmental Disabilities (OPWDD) Division of Service Delivery Regional Offices seeks a Developmental Disabilities Program Specialist (DDPS)1 within the Age Out Unit of DDRO Region 4. The Age Out Unit is responsible for transitioning OPWDD eligible individuals in residential schools and in foster care to adult residential and day services. The DDPS 1 plays a critical role within the Age Out Unit’s planning process.

PRIMARY DUTIES AND RESPONSIBILITIES:

The candidate selected for this position will act as the Residential School Transition Coordinator (RSTC) and line supervisor. The incumbent’s primary duties and responsibilities will include but are not limited to the following:
• Function as a line supervisor for the Age Out Unit.
• Ensure that individuals aging out of residential schools receive appropriate OPWDD adult services in a timely fashion. Bring any issues and concerns to leadership.
• Participate in weekly and monthly meetings, both in person, by phone or by video conference with agencies, families, residential schools and developmental staff, all meetings relate to the transition of individuals from residential settings to OPWDD adult services
• Work closely with the Eligibility Team to ensure those in residential schools meet OPWDD criteria to receive services
• Maintain dialog with families, both in writing and over the phone regarding their child and the placement process
• Send referrals for residential placement and Day Services to appropriate agencies
• Will cover assigned cases within NYC Region 4
