Minimum Qualifications Candidates must be reachable for appointment on Current Civil Service Eligible List (List #37-810 – Promotion or List #26-229 – Open/Competitive should the Promotion List result in fewer than 3 qualified and interested candidates) for this title.
OR
Candidates must have one year of permanent service as a Social Work Assistant 3 to be eligible for lateral transfer.
OR
have one year of permanent service in a title eligible to transfer via Section 70.1 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.

PREFERRED QUALIFICATIONS:

• A bachelor’s degree in a human service-related field (i.e., Social work, psychology, etc.) and minimum of two (2) years’ experience working with people who have Developmental Disabilities.
• Display strong writing, verbal communication, interpersonal, and organizational skills.
• Possess the ability to work independently.
• Have a working knowledge of and be proficient in the use of a computer.
• Possession of a valid driver’s license to operate a motor vehicle in New York State is preferred.

Duties Description This Social Worker Assistant 3 (SWA 3) position is a critical Health, Safety & Welfare position that is needed to ensure oversight of the Family Care program.
The SWA 3 Supervisor functions as a Family Care Home Liaison and a Supervisor. The SWA 3 may need to cover over 35 individuals living in over 25 Family Care Homes in 4 counties. The SWA 3 will ensure that individuals receive their benefits as delineated by regulations. The SWA 3 would monitor Family Care Home Liaisons to ensure that the living environments meets the individuals physical, medical, social, and spiritual needs. This position would also ensure that the physical plant of the Family Care Home’s comply with OPWDD regulations. This staff will be supervised by the Family Care Social Worker 2 (LMSW2).

PRIMARY DUTIES AND RESPONSIBILITIES:

• It will be the function of the SWA 3 to monitor 11-20 homes to ensure that safety and health issues are immediately address and the required follow up is completed.
• The Social Worker Assistant 3 is responsible for ensuring that the Family Care Providers are adhering to the Family Care policies relative to the health and safety of over individuals who reside in Westchester and Rockland county.
• Monitor Family Care Homes to ensure living environments meets the individuals physical, medical, social, and spiritual needs
• Responsible for ensuring that the health and safety of the individuals are maintained in the Family Care Home.
• Monitor and develop Staff action, safeguards and all billable documents and ensure that they are consistent with individual life plan as per OPWDD regulation.
• Ensure communication occurs between the Family Care Providers and the other members of the treatment team
• Acts as a liaison between the Family Care Provider, the individual and the other treatment team members
• Ensure that the Family Care Providers are following regulatory requirements.
• Ensure that individuals receive their benefits as delineated by regulations
• Ensure that Family Care Providers are adhering to the individual plans of care
• The SWA 3 will be certified as a trainer and will provide Mandatory American Heart Association First Aid and CPR training to Family Care and Respite Providers.
• Other duties as assigned
