OR

Transfer Opportunity - Candidates must have one year permanent competitive or 55-b/c service in this title and be eligible for lateral transfer; OR be eligible for transfer via Section 70.1 of Civil Service law. To qualify for Section 70.1 transfer, candidates must be licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) and have at least one year of permanent competitive or 55-b/c service in a title approved for transfer by Civil Service within two salary grades of the Grade 20 level.

OR

Temporary Appointment – In the event there is not a valid eligible list at this location, candidates who meet the minimum qualifications for the Licensed Master Social Worker 2 exam may submit a resume and if hired, will be appointed with temporary status.

Minimum Qualifications:

Either A. licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) AND have one year of post NYS licensure professional social work experience; **

Or B. licensed and currently registered by New York State as a Licensed Clinical Social Worker (LCSW).
Registration and licensure as a Licensed Mental Health Counselor is not considered qualifying for these examinations.

** Professional social work experience involves activities such as screening, assessing and diagnosing individuals' and families' psychosocial needs, strengths and limitations; participating in the development of treatment plans and discharge plans; counseling and educating individuals, groups and families to promote mental, emotional, physical and social functioning; helping access needed services; providing client/patient advocacy and crisis intervention; and developing, coordinating and evaluating services and support systems. Experience limited to social work research or social work administration is NOT considered qualifying.

Duties Description POSITION DESCRIPTION:

Under the supervision of the Developmental Disabilities Program Specialist 2, the Residential School Transition Coordinator (RSTC) plays an integral role in facilitating planning for youth aging out of residential schools. The RSTC is responsible for coordinating the transition to adult OPWDD services for students aging out of residential school settings.
The RSTC will be expected to assist with this planning for students from across the 18 counties within the Hudson Valley, Taconic, and Capital Districts of Region 3.
Across the 18 counties within Region 3, approximately 147 students will aging out from these settings in 2022-2024 . This number is expected to grow as additional students are found to be eligible for OPWDD services.

DUTIES:

• Will have continuous interface with residential schools, families/guardians, the individual, Central Office and provider agencies around the supports needed for transition from the residential school.
• Assists with person centered planning and ensuring connections to residential or in-home supports as well as ensuring identification of day services.
• Responsible for monitoring and upkeep of the residential age out data base.
• Participate in quarterly meetings for individual’s residing in residential schools to ensure awareness of current needs.
• Visit individuals at their residential schools to be familiar with theirs needs related to planning.
• May be needed to participate in Fair Hearings.
• Facilitate the sharing of information for youth aging out of residential schools with the Certified Residential Opportunities Committee.
• Involved in efforts to identify providers to plan and develop services including but not limited to, presentations at the Residential Opportunities Committee and involvement in the Letter of Interest (LOI) process.
• Ensure the required Preliminary Adult Service Plans are submitted timely and reviewed to ensure they meet the individual’s needs.
• Assist with completing Developmental Disabilities Profile 2 (DDP2) Assessments
• Assist with reviewing Higher Needs Funding applications

• Respond to requests for information from Leadership within time frames given.
• Complete other duties requested by DDPS2, DDPS4 and Deputy Director.
