Job Specifics

Duties Description Are you interested in starting your career as a Family Health Nurse Practitioner with New York State? (Facility Name) is recruiting a Nurse Practitioner Family Health to provide patient-centered primary health care to individuals with mental illness. Candidates from outside or within State Service can apply.

As Nurse Practitioner Family Health, you will provide primary health care including:
• Conducting physical examinations.
• Diagnosing and treating health conditions.
• Establishing individual goals and treatment objectives in conjunction with treatment team.
• Prescribing medications and treatments, monitoring impact, and modifying as needed.
• Maintaining medical records and documentation.

Minimum Qualifications Candidates from outside or within State Service may be considered for hire under the Hiring for Emergency Limited Placement - Statewide (NY HELPS) program as a Nurse Practitioner Family Health if they have certification and current registration as a Nurse Practitioner in the Family Health specialty practice area by the New York State Education Department.
OR
Candidates from within State Service can be considered for a competitive class appointment if they have one year of permanent competitive or 55-b/55-c service in a title eligible for transfer under Section 70.1 of the Civil Service Law.

Additional Comments:
Posted Salary is the sum the statutory job rate for PS&T Grade 24 $88,639, plus an Increased Hiring Rate of $23,258 and a Geographic Differential of $25,000.

1. If you fail to maintain your license and/or registration once you are appointed, action will be taken to remove you from the position.

2. In order to be eligible for appointment and to maintain employment, you cannot be listed as an excluded individual or entity on any of the Federal and/or State Medicaid and Medicare exclusion lists (or excluded from any other Federal or Federally assisted program). If you are appointed and subsequently listed as an excluded individual or entity on any of these lists (or excluded from any other Federal or federally assisted program), you may be terminated from your employment.

If appointed you may be required to become an enrolled Medicare provider; obtain and provide to your employer a National Provider Identifier (NPI) number issued by the National Plan and Provider Enumeration System (NPPES); and otherwise actively participate to the degree necessary to allow for your services to be billed through Medicare and Medicaid. If you are appointed and you lose the ability to bill through Medicare and Medicaid, you may be terminated from your employment.

4. Your degree or college credit must have been awarded by a regionally accredited college or university or one recognized by the NYS Education Department as following acceptable educational practices. If your degree or college credit was awarded by an educational institution outside the United States and its territories, you must provide independent verification of equivalency. You can write to the Examination Information Desk of the Department of Civil Service for a list of acceptable companies who provide this service. You must pay the required evaluation fee.
