Minimum Qualifications: For qualified employees of NYS: On or before the date of the written test, you must have had one year of permanent competitive or 55-b/55-c service as a Social Work Assistant 2, Social Work Assistant 2 (Spanish Language), or Social Work Assistant 2 (Manual Communications).
For all other qualified individuals: Either 1. a bachelor’s degree or higher in a human service area* and one year of permanent competitive 55-b/55-c service as a Social Work Assistant 2 or parenthetic;
Or 2. three years of permanent competitive 55-b/55-c service as a Social Work Assistant 2 or parenthetic.
*For the purpose of these examinations, degrees in the following human service areas are considered qualifying: the professional disciplines of art therapy, audiology, dance therapy, drama therapy, music therapy, nursing, nutrition, occupational therapy, physical therapy, psychology, social work, speech/language pathology, and therapeutic recreation. Also included are related academic disciplines associated with the study of: human behavior (e.g., gerontology, human services, sociology, speech communication); human skills development (e.g., counseling, education, human development); or any other study of services related to basic human care needs (e.g., community mental health, rehabilitation counseling).

For all other qualified individuals, on or before the date of the examination, candidates must have either:
A. a master’s degree in social work;
OR
B. a bachelor’s degree in social work and one year of post-bachelor’s social work experience;
OR
C. a bachelor’s degree or higher in a human service area and two years of post-bachelor’s social work experience.

Duties Description POSITION DESCRIPTION/PRIMARY JOB DUTIES:

Under the supervision of the DDRO Region 2 DDPS I Self Direction (SD) Team Leader, the candidate will perform the role of Self-Direction Specialist for the six counties of the Sunmount region. The primary function of this role is to assist people with developmental disabilities in enrolling in and maintaining a Self-Direction Plan to ensure provision of their services in the most person-centered manner possible.

Duties include (but are not limited to):

• Provide daily support, training and technical support to Voluntary Provider Agencies in all matters relating to the Self-Direction Program
• Review and approve Self-Direction Budgets
• Provision of trainings as needed and assisting individuals, families and staff to ensure timely provision of services
• Be a member of the Self-Direction team – tracking Self-Directed Budgets & Services
• Assist in processing of individuals seeking to Self-Direct their supports and services
• Follow Self-Directed Guidance and provide technical support
• Become knowable of ADM (Administrative Memorandum) of Self-Directed Services
• Work with Region 2 Self-Direction Liaisons & statewide liaison
• Facilitates Self-Direction and Front Door meetings and trainings
• Provide training and technical support to Brokers/Fiscal Intermediary on the Person Centered Planning Self-Direction template and other related training needs
• Work with Self-Direction Team in maintaining current Self-Direction Plans.
• Assist in plan modifications and revisions
• Facilitate broker training – provide daily technical support to brokers
• Act as a liaison to families, Care Managers, agencies and Fiscal Management Serve in the provision of Individual Community Supports
• Outreach for new brokers
• Outreach to Care Managers families and provider agencies so that they are familiarized with Self-Direction and the process to obtain other Self-Directed services
• Member of the Quality Review Team
• Be subject to additional duties related to the Managed Care Transition
• Regular travel within the six-county district, and on occasion outside the district for trainings, meetings and other assignments
• Other duties as assigned
