Minimum Qualifications MINIMUM QUALIFICATIONS:
Candidates must be currently in title, eligible for 70.1 transfer, reachable on current Civil Service Eligible List for Social Work Assistant 3. Provisional appointment may be considered. To be eligible for provisional appointment candidates must have either:
• a Master’s degree in social work; OR
• a Bachelor’s degree in social work AND one year of post-Bachelor’s experience social work* OR
• a Bachelor’s degree or higher in a human services area AND two years of post-Bachelor’s experience in social work; **OR
• be a qualified employee of New York State and have either:
1. a bachelor’s degree or higher in a human service area* and one year of permanent competitive 55-b/55-c service as a Social Work Assistant 2 or parenthetic; Or
2. three years of permanent competitive 55-b/55-c service as a Social Work Assistant 2 or parenthetic.
*Degrees in the following human service areas are considered qualifying: the professional disciplines of art therapy, audiology, dance therapy, drama therapy, music therapy, nursing, nutrition, occupational therapy, physical therapy, psychology, social work, speech/language pathology, and therapeutic recreation. Also included are related academic disciplines associated with the study of: human behavior (e.g., gerontology, human services, sociology, speech communication); human skills development (e.g., counseling, education, human development); or any other study of services related to basic human care needs (e.g., community mental health, rehabilitation counseling).
Examples of non-qualifying degrees include but are not limited to: anthropology, archeology, communication, criminal justice, fine arts, philosophy, and political science.
**Qualifying social work experience involves activities such as assessing individual and family needs, strengths and limitations; assisting in the development of treatment plans; providing counseling to promote mental, emotional, physical and social functioning; helping individuals and families access supportive services; coordinating and evaluating services, programs and support systems; and providing support, advocacy and referral as appropriate. Examples of non-qualifying experience include providing direct hands-on care to patients/consumers by assisting with bathing, dressing, mobility, eating, and other activities of daily living; transporting/accompanying patients/consumers to appointments or activities; planning, organizing and participating in recreational activities for patients/consumers, such as sports, games, creative arts, and/or clubs; and performing safety and/or security related functions for a facility.

Candidates must possess a valid Driver’s License.

Duties Description POSITION DESCRIPTION:
As a Social Work Assistant 3 this person will manage a complex caseload and exercise a high degree of independence. They will perform a variety of case management and service coordination activities for an assigned caseload; interview individuals, their families, and/or others to gather background information, prepare case histories, and determine eligibility for services; advocate on behalf of and assist individuals in identifying and obtaining services; work with interdisciplinary treatment teams and participate in the development and implementation of treatment and service plans; coordinate services between agencies, programs, and providers; and follow-up to ensure individuals’ needs are met.

PRIMARY DUTIES AND RESPONSIBILITIES:
• Treatment Coordination
• Maintain clinic records to ensure documentation and authorization for services are up-to-date and records purged as needed.
• Conduct semi-annual and annual reviews for all assigned individuals per Article 16.
• Conduct intake for referrals and obtain necessary documentation for Article 16 Clinic
• Maintain relationships and ongoing contact with approved service providers and referral sources
• Ensure approved service providers complete appropriate treatment plans and maintain semi-annual treatment notes per Article 16.
• Monitor service provision/Medicaid irregularities and report concerns to Article 16 administrator.
• Complete appropriate termination paperwork when a clinic case is closed.
• Maintain referral information for Article 16 clinic.
• Maintain current lists of individuals receiving Article 16 services, track and monitor reviews and admission/termination of services.
• Conduct periodic record reviews for quality assurance within Article 16 Clinic.
• Send out and process satisfaction surveys and other required Article 16 forms.
• Assist as needed and assigned in outlying psychiatry clinics, including scheduling follow-up appointments.
• Other duties as assigned.
