Job Specifics
Minimum Qualifications Must currently be a permanent Community Mental Health Nurse.
OR
Must currently be active and reachable on the Community Mental Health Nurse Civil Service List, and meet the educational requirements as specified on the exam announcement.
OR
Must be Eligible for a 70.1 Civil Service Law transfer, possess a license to practice as a Registered Nurse in New York State and meet the specified education and experience criteria as noted on the currently Civil Service exam announcement for Community Mental Health Nurse.
OR
Must currently qualify for provisional* appointment to Community Mental Health Nurse.
NOTE: *Provisional Appointments will only be considered if there are insufficient in-Title and/or Civil Service List Candidates.

? Although an office is in Orange County, the actual assignment will require working throughout Orange & Sullivan Counties therefore must be willing to travel.
Duties Description • Participates in plans to consider moving individuals to Family Care by ensuring that health needs can be adequately addressed in a particular home.
• Reviews health care documentation prior to movement to ensure that the physical examination, and all other appropriate examinations, and/or assessments are completed.
• Visits the Family Care home on no less than a quarterly basis to ensure that health services outlined in the service plan reflect individual’s needs, and that such needs are being adequately addressed. These visits must be documented by the nurse.
• Reviews, and has knowledge and understanding of the regulations, policies and procedures for Family Care and supports the individual’s increased independence in identifying health care needs and accessing services.
• Reports concerns of safety issues to the Family Care Coordinator, Family Care Home Liaison, Medicaid Service Coordinator, and/or other administrative staff as necessary.
• Acts as a mentor and consultant to Family Care providers empowering them to the skills and knowledge sufficient to allow them to interface effectively with most health care professionals and to act as health care advocates.
• Ensures that all medication is prescribed, obtained, provided, received, administered, safeguarded, documented, refilled, and/or disposed of in a manner that ensures the health, safety and well-being of the individuals being served. New York Codes, Rules and Regulations {14 NYCRR Section 633.17(a)(7)}.
• Ensures that information is provided to the individual, advocate, Family Care provider, and other providers about the condition for which a medication is to be used, and the specifics regarding administration {14 NYCRR Section 633.17(a)(13)(ii)}
• Ensures that a review of the individual’s medication regime is conducted on no less than a semiannual basis {14 NYCRR Section 633.17(18).
• Ensures that a current nursing assessment is available as a basis for health care planning, and includes in the assessment an evaluation of the individual’s ability to self-administer medications.
• Ensures that the individual is referred for routine and/or specialized health visits as needed and that such services are provided in a timely manner.
• Refers medical concerns to the physician and/or the physician’s assistant, and where warranted, the Family Care provider with any concerns related to admitting the individual to the hospital, and/or alternate other available options.
• Provides information and assistance as needed to Family Care providers that will provide the necessary health supports to individuals. This may include ensuring that Family Care providers access Tuberculosis (TB) testing and infection control information.
• Ensure that the individual and/or advocate is made aware, if possible, of this or her current health status and medical plan of care, with full knowledge of the proposed risk and benefits associated with the plan along with its reasonable alternatives.
• Provides training as needed for individuals and Family Care providers to enable them to access and/or provide appropriate care.
• Ensures that the individual’s rights to expect reasonable continuity of care and privacy concerning his or her own health care and confidentiality of all communication and health records are being safeguarded.
• Actively supports the individual’s rights to consideration and respective health care, as well as making every effort to render support to making individual decisions as consumers of health care consistent with the individual’s goals, values, preferences and lifestyles.
• Requests and reviews a copy of consultation reports and all other pertinent medical documents, specialty medical services, etc.

• The assignment of PS&T employees will be made in accordance with the Memorandum of Understanding between PEF and Hudson Valley DDSO regarding the posting of job vacancies.
