Job Specifics

Minimum Qualifications Public candidates: Active list candidate on the New York State Department of Civil Service’s Professional Career Opportunities Select Titles eligible list #50-001 with a score of 100.0 or higher OR qualified 55B/C candidate in possession of a bachelor’s degree (or higher) in health sciences, natural sciences, social sciences, or public health.

Preferred Qualifications: Experience in communicable disease epidemiology and disease surveillance activities, especially in a prison setting. Strong written and verbal communication skills. Ability to work independently. Experience using NYSDOH data systems, such as the Electronic Clinical Laboratory Reporting System (ECLRS) and the Communicable Disease Electronic Surveillance System (CDESS).

Permanent transfer candidates: Current Department of Health (DOH) employee with permanent or contingent-permanent status as a Public Health Field Services Representative 1 (G18); OR current NYS employee with one year or more of permanent or contingent-permanent, competitive service in a title eligible to transfer under Section 70.1, or Section 70.4 of the Civil Service Law to a Public Health Field Services Representative 1 (G18), a Public Health Field Services Representative Trainee 1 (NS), or a Public Health Field Services Representative Trainee 2 (NS). Duties Description Locate, interview, and document individuals exposed to and living with HIV and STIs (e.g., Syphilis, Gonorrhea, Chlamydia); advise individuals of possible infection, prevention, care, and treatment; confirm that individuals receive effective prevention, care, and treatment services.
Conduct program promotion and outreach; provide technical assistance and education to individuals, providers, and the community.
Conduct disease screening at point of contact during field visits and at various testing venues with clients for whom testing is indicated.
Develop and submit reports documenting program activities, outcomes, emerging challenges, and other information.
Participate in limited-scope investigations of disease outbreak and/or transmission sites to ascertain the extent of outbreak and/or transmission and recommend response.
Represent the disease intervention service program at various public health forums, community meetings, conferences, and other events.
May assist in the training of other PHFSRs, particularly trainees.
Respond to public health emergencies and participate in preparedness-related activities, training, and exercises as needed.

Additional Comments A downstate adjustment of $3,026 will be added to your salary.
