Minimum Qualifications MINIMUM QUALIFICATIONS FOR PERMANENT APPOINTMENT:
• Currently serving as a LMSW 1 (Spanish Speaking) and eligible for transfer OR
• Reachable on the Civil Service List for Licensed Master Social Worker 1 (Spanish Language) OR
• Eligible for 70.1 transfer per Civil Service Rules and Regulations.
MINIMUM QUALIFICATIONS FOR PROVISIONAL APPOINTMENT:

• Candidates must be licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).

Duties Description • Supervise Family Care Program through the delivery of comprehensive habilitation services provided by multi-disciplinary team members.
• Will work as part of an interdisciplinary team to accurately identify the comprehensive needs of an individual, design, implement and document appropriate services in EHR.
• Ensure Family Care Providers receive training and maintain certification.
• Maintain certification according to regulatory requirements for Family Care and HCBS Res Hab
• Ensure that the FC Programs meet all applicable Federal, State and local regulations.
• Interface and facilitate coordination of Family Care Screening Process with Families, CCO’s and DDRO for individuals who are interested in pursuing FC services.
• Liaison with natural families, Due Process for movements, respite as needed.
• Provide specialized training, when required, and guidance to staff /FCP on a day-to-day basis.
• Ensure community inclusion activities for each Individual and the delivery and documentation of community inclusion services in accord with each Individual’s “valued outcomes”.
• Ensuring the appropriate and timely reporting of family care incidents, including the completion of required forms, and required follow-up.
• Ensure the completion agency assigned investigations and investigative reports (and follow-up when required) for each reportable incident.
• Work closely with the residential and clinical team to ensure coordination of services for Family Care Individuals.
• Liaison with Voluntary Operated Family Care Agencies in the certification and provision of services to Individuals living in voluntary operated Family Care homes.
• Meets with and instructs staff regarding work projects, program objectives, delivery of services techniques and the implementation of OPWDD policy.
• Establishes standards of performance, observes staff visits and activity, determines the efficiency of the staff and programs being delivered.
• Reviews Individuals records to determine if proper and adequate information is continuously updated.
• Participates in meetings with various DDSO and Agency clinical/support staff in order to provide and exchange program and/or administrative information.
• Responds to crisis situations and emergencies involving Individual’s and provides leadership to assist staff and Individuals during such times.
• Support implementation and development of Individual Habilitation Plans, Review of Res. Hab. Data sheets, establishment of goals, and monitoring Individuals’ progress toward the achievement of those goals.
• Ensure the implementation of services as per Individual Life Plans and Family Care Providers
• Make validation survey visits, Observe and review fire drills as needed.
• Communicate and review all pertinent information with Supervisor in a timely manner.
• Additional duties as deemed necessary by the Unit TTL or Agency Director.
