Minimum Qualifications MINIMUM QUALIFICATIONS FOR PERMANENT APPOINTMENT:

? Reachable on the Civil Service List for Social Work Assistant 3; OR
? Currently serving as a permanent Social Worker Assistant 3 and eligible for transfer; OR
? One year of permanent competitive service in a title eligible for transfer under Section 70.1 of the NYS Civil Service Law.

MINIMUM QUALIFICATIONS FOR PROVISIONAL APPOINTMENT: If appointed on a provisional basis, you must participate in the next Civil Service examination(s) and be a reachable candidate in order to obtain permanent status and maintain your position.

Candidates must be a qualified employee of New York State and have had three months of permanent competitive or 55-b/55-c service as a Social Work Assistant 2, Social Work Assistant 2 (Chinese Language), Social Work Assistant 2 (Korean Language), or Social Work Assistant 2 (Spanish Language).
Only the title(s) listed is qualifying for this examination(s).

Either 1. a bachelor’s degree or higher in a human service area* and one year of permanent competitive 55-b/55-c service as a Social Work Assistant 2 or parenthetic;

Or 2. three years of permanent competitive 55-b/55-c service as a Social Work Assistant 2 or parenthetic.
*For the purpose of these examinations, degrees in the following human service areas are considered qualifying: the professional disciplines of art therapy, audiology, dance therapy, drama therapy, music therapy, nursing, nutrition, occupational therapy, physical therapy, psychology, social work, speech/language pathology, and therapeutic recreation. Also included are related academic disciplines associated with the study of: human behavior (e.g., gerontology, human services, sociology, speech communication); human skills development (e.g., counseling, education, human development); or any other study of services related to basic human care needs (e.g., community mental health, rehabilitation counseling).
Examples of non-qualifying degrees include but are not limited to: anthropology, archeology, communication, criminal justice, fine arts, philosophy, and political science.

Duties Description • Work closely with the residential clinical team to ensure coordination of services for Family Care.
• Maintain certification according to regulatory requirements for Family Care and HCBS Res Hab.
• Liaison with Voluntary Operated Family Care Agencies in the certification and provision of services to Individuals living in voluntary operated Family Care homes.
• Ensure Family Care Providers receive training and maintain certification.
• Meets with and instructs staff regarding work projects, program objectives, delivery of services techniques and the implementation of OPWDD policy.
• Establishes standards of performance, observes staff visits and activity, determines the efficiency of the staff and programs being delivered.
• Reviews Individuals records to determine if proper and adequate information is continuously updated.
• Participates in meetings with various DDSO and Agency clinical/support staff in order to provide and exchange program and/or administrative information.
• Responds to crisis situations and emergencies involving Individual’s and provides leadership to assist staff and Individuals during such times.
• Make validation survey visits, Observe and review fire drills as needed.
• Supervise Family Care Program through the delivery of comprehensive habilitation services provided by multi-disciplinary team members.
• Support Home Liaisons and/or clinical staff in the development of Individual Habilitation Plans, implementation, Review of Res. Hab. Data sheets, establishment of goals, and monitoring Individuals’ progress toward the achievement of those goals. Ensure that the programs meet all applicable Federal, State and local regulations.
• Ensure the implementation of Individual Life Plans by Home Liaisons and Family Care Providers.
• Provide specialized training, when required, and guidance to staff on a day-to-day basis.
• Ensure community inclusion activities for each Individual and the delivery and documentation of community inclusion services in accord with each Individual’s “valued outcomes”.
• Ensuring the appropriate and timely reporting of family care incidents, including the completion of required forms, and required follow-up.
• Ensure the completion agency assigned investigations and investigative reports (and follow-up when required) for each reportable incident.
• Communicate and review all pertinent information with Supervisor in a timely manner.
• Additional duties as deemed necessary by the Unit TTL or Agency Director.
