*For the purpose of these examinations, degrees in the following human service areas are considered qualifying: the professional disciplines of art therapy, audiology, dance therapy, drama therapy, music therapy, nursing, nutrition, occupational therapy, physical therapy, psychology, social work, speech/language pathology, and therapeutic recreation. Also included are related academic disciplines associated with the study of: human behavior (e.g., gerontology, human services, sociology, speech communication); human skills development (e.g., counseling, education, human development); or any other study of services related to basic human care needs (e.g., community mental health, rehabilitation counseling).

Duties Description POSITION DESCRIPTION:
This position will be assigned to the Regional Office Region 4 Front Door Team. As such will assist individuals and families seeking and/or amending OPWDD services from all 5 boroughs. Will be under direct supervision of a Front Door lead and be a part of the Region 4 service access function.

PRIMARY DUTIES AND RESPONSIBILITIES:

Staff assigned to the Front Door division at the Regional Offices serve in a variety of capacities including, but not limited to:
Care Coordination Service Liaison (CCSL)
• Will process CCO2 submissions in CHOICES. This includes reviewing documentation submitted (DOH and DOC forms, certified return receipt, notes/letters from CCO, etc.), contacting CCO and/or family, sending out 30-day notice letters, entering notes in CCO2, and completing the DDRO verification form. Submit CCO1/2 override request to Central Office.
• Will troubleshoot general CM and individual/family issues including, but not limited to, linking CCOs to appropriate DDRO contacts, navigating CHOICES, communication with stakeholders via phone and email, and sharing community resources/contacts.
• Will complete the Customer Service Log for Central Office.
• Will facilitate local Care Manager training to address local needs.
• Will issue Code 95 removals when individuals are transitioning out of waiver. Obtain letter from individual/family and signed DOC form. Check TABS if known to OPWDD/active services. Close out of programs first using CE-5, then send to RSFO. TBD on who's doing what with Enrollment. Redirect individuals seeking Code 95 adds to Front Door.
• Reviewing and processing approvals for SFAR (NMCM, state-paid services such as respite and CH).

Front Door Facilitator (FDF)
• Will serve as the liaison to the families seeking to obtain services.
• Will assist with conducting information sessions, send out eligibility documents, conduct assessments, complete pre-authorizations, consult with Care Coordination Organizations, and complete and mail authorization forms.
• Will address all issues that may arise as families begin and transition throughout all the various processes involved in establishing eligibility and obtaining services.
• Will interface with hundreds of newly identified families per month through required sessions that introduce them to OPWDD services.
• Will continue oversight to the families as they progress through the authorization process.
• Will continue to provide ongoing assistance to the families who have not yet completed the Front Door process.

Children’s Waiver
• Review all incoming Children's Waiver (CW) request for annual LCED redeterminations and complete LCED’s in OPWDD’s system. Request additional information as necessary to complete the LCED.
• Uploaded completed LCED redeterminations into the data systems of OPWDD (TABS/Choices/Logs) and DOH (UAS and Secure SFT). Notify providers when LCEDs are completed and if individuals no longer meet OPWDD eligibility.
• Assist CW/DOH staff with initial OPWDD Eligibility Request. Review the incoming eligibility packet and assure documents are appropriate. Advise agency if anything is missing. Upload Eligibility packets into Choices then inform the Eligibility Unit when an initial or provisional packet is ready for review. Continue to follow up on all eligibility cases that have been submitted until a determination has been rendered. Maintain the CW eligibility logs. Serve as the go between for CW and Eligibility team.
• Periodically notify the CW agencies of expired LCED’s and aging out individuals. Track progress of assigned agencies concerning LOCEDs. Assist in TABS clean up as assigned by supervisor or Central Office. Run Children’s waiver reports in TABS to monitor assigned caseload.
• Lead out in the transfer process between OPWDD and DOH/CW by reviewing and processing incoming requests for transfer. As the CW Liaison coordinate and assure that the transfer process is followed by all providers. Provide in-service, technical guidance and conduct meetings when clarification and problem resolution is needed concerning the transfer process, and resources. Update and maintain transfer logs.
• Work closely with OPWDD Central Office CW leads and, Region 4 Eligibility, Front Door, Waiver, ACS/Aging Out Units to ensure smooth transfers to OPWDD services. Interface directly with DOH, and Children Waiver Providers, CCO’s, in resolving any issues that may arise. Support all members of the team to assist in completing transfers. Be well versed in CW program, the Front Door and Eligibility processes (EAA, DDP2, Waiver documents, RSA, Trainings, and eligibility steps and standards.)
• Work collaboratively with the DOH leaders and Central Office to ensure proper codes have been removed and added to finalize transfers between programs.
• Interface with OPWDD Aging Out/ACS Coordinators and other stakeholders when CW cases need to transfer while simultaneously moving into residential placements. Attend necessary meetings.
• Provide guidance and to families, CW agencies, CCO team members and OPWDD staff on the difference in the two programs. Assist in the referral process between these programs as per the CW manual.
• Take an active part in the OPWDD CW Community of Practice, CW meetings, calls and in-services. Complete all required specialized DOH CW trainings via DOH and well as regular OPWDD Trainings. Incumbent must develop a strong working Knowledge of both OPWDD and CW services and processes. Incumbent will work in both OPWDD and CW e-mail, trainings, and data base systems.
• Complete special projects and other duties as assigned. Be flexible as the program changes.
• Must be proficient in the use of SharePoint, Teams, Microsoft Word, Access, and Excel.
