Minimum Qualifications Permanent Transfer Candidates: Current NYS Department of Health employee with permanent or contingent-permanent, competitive status as a Health Systems Specialist 4, G-25 OR a NYS employee with one year or more of permanent or contingent-permanent, competitive service in a title at or above a G-23 and deemed eligible for transfer to Health Systems Specialist 4, G-25 under Section 70.1 of the NYS Civil Service Law. 70.1 source titles include Consultant Nurses, Principal Sanitarian, Health Program Administrator 1, Standards Compliance Analyst 2.

Provisional Candidates: Current NYS Department of Health employee with one year permanent or contingent-permanent, competitive status as Community Nursing Services Consultant (Home Health Services), Consultant Nurse (Hospital Nursing Services Administration), Consultant Nurse (Community Nursing and Home Health Services), Health Systems Specialist 3, Health Systems Specialist 3 (Emergency Medical Care), Hospital Nursing Services Consultant, Principal Sanitarian, Regional Medical Care Administrator, or Senior Nutrition Services Consultant, AND sat for exam 38-516, held October 16, 2021.


Preferred Qualifications: The incumbent should have a strong clinical background and/or knowledge of the federal and state survey process, including the ASPEN system.

Duties Description The Health System Specialist 4 will serve as the Western Regional Office Rochester Program Manager in the Long Term Care Unit. Duties will include, but are not limited to; ensuring all state and federal mandated activities related to nursing home surveys, complaints, certificates of need, FOIL requests and court related activities and pre-openings are completed appropriately and in a manner consistent with statute, regulation, policy and procedure; ensure that survey staff receives in-service training related to nursing home program policies and procedures and has resources available to implement them appropriately; ensure that all required performance appraisals are completed on a timely basis, coordinate and monitor the complaint investigation time lines with the nursing home survey cycle and keep the LTC Program Director apprised of the status of both programs; provide QA oversight in conjunction with WRO personnel to ensure regulations are consistently enforced in the region; function as a liaison with Albany Central Office QA regarding interpretations of the regulations, as well as the survey process to ensure statewide consistency, coordinate information and gather data to facilitate monthly statistical analysis necessary for the surveillance initiative; and assure accurate input into systems, such as ASPEN Central Office (ACO) and ASPEN Complaint Tracking System (ACTS) databases, respond to public health emergencies and participate in preparedness related activities, training and exercises as directed and needed. The Health System Specialist 4 will be available after normal business hours to respond to emergencies, and will serve as Administrator on Duty for a regular assignment.

Weekends and off hour work may apply. Candidates for Nursing Home Surveillance positions must pass SMQT(Surveyor Minimal Qualifications Test) as required by the Centers for Medicare and Medicaid Services to conduct surveys. Training will be provided prior to scheduling of examination.
