Minimum Qualifications Permanent Transfer Candidates: Current DOH employee with permanent or contingent-permanent competitive status as a Health Care Surveyor 4, G-24 OR current NYS employee with one year of permanent or contingent-permanent competitive service in a qualified title and eligible for transfer to Health Care Surveyor under Section 70.1 of the Civil Service Law OR eligible for transfer under Section 70.4 of the Civil Service Law (NYS employee with one year of permanent or contingent-permanent competitive service at G-22 or higher and meeting the open competitive minimum qualifications listed below).
Open Competitive: possession of a license and current registration to practice nursing in
New York State; a bachelor’s degree in nursing; and four years of post-licensure, specialized professional administrative or professional clinical experience. One year of the experience must have been at a supervisory level.
Preferred Qualifications:Knowledge of Nursing Home Surveillance process and State and Federal Regulations. Strong leadership skills in management of professional and administrative support staff. Participation in quality assurance and performance improvement initiatives, staff training/development. Excellent investigative, critical thinking and problem solving skills. Scheduling experience preferred. High profiency in written and verbal communications. Proficient in Microsoft Office Suite of applications, Adobe, electronic filing and data transfer and uploading. Ability to multi-task, work independently and prioritize as required. Surveyor Minimum Qualifications Test (SMQT) certified.
Duties Description The Health Care Surveyor Nurse 4 will schedule surveys and complaint investigations in accordance with Survey and complaint managers under the direction of the Regional Area Director. Review investigative reports and statement of deficiencies, facility plans of correction, and post survey activities. Recommend enforcement actions. Identify training and performance initiatives for staff. Provide field supervision of surveillance staff. Participation in quality assurance meetings to achieve Statewide standards and consistency; identify training needs and coordinate in-service education for clinical and support staff to continually improve knowledge and performance; supervise, guide and act as a resource to survey teams in the performance of surveillance activities to determine compliance with Federal, New York State and Local Health and Life Safety Code regulatory requirements; clarify survey protocols for program staff and providers Participate in recertification surveys and complaint investigation according to program needs. Provide clinical and survey process guidance to staff.
Additional Comments Full time, permanent appointment. Occasional statewide travel, including overnights and travel to areas that may not be served by public transportation is required. Current and continued registration with the NYS Education Department to practice as a Registered Nurse is required for employment.

**Posted salary range includes the statutory hiring rate and job rate for PS&T Grade 25 ($88,161.00 $11,200.00) and an additional Geographic Pay Differential of $20,000
