Minimum Qualifications Permanent Transfer Candidates: Current NYS employee with one year of permanent or contingent permanent competitive service as a Health Care Surveyor 3, G22, or Community Nursing Services Consultant Home Health Services, G22 and eligible for transfer under Section 70.1 of the Civil Service Law OR Current NYS employee with one year of permanent or contingent permanent competitive service in a title G20 or higher and meeting the open competitive minimum qualifications listed below.
Open Competitive Minimum Qualifications: Currently licensed and registered as a professional nurse in NYS; AND a minimum of five years of post-licensure health care, public health, community health, or other nursing experience in an appropriate facility setting. Three years of this experience must have been providing direct patient care in a licensed health care facility, and two years of this experience must have been in nursing supervision or working in the capacity of a nurse administrator.
Preferred Qualifications: The preferred candidate must have strong interpersonal, problem solving, decision-making and time management skills and the ability to communicate effectively with all internal and external stakeholders. In addition, the ideal candidate possesses knowledge of pediatric diagnoses and diseases that impact children's development and activities of daily living, and has experience with pediatric home health care and related medical supports and supplies.
Duties Description This position will serve as a clinical expert for policy development, program implementation and monitoring for the Children’s Health Home program, 29-I Health facilities, Home and Community Based Services (HCBS) Children’s Waiver for children/youth with behavioral health, medically complex, and developmental disabilities need. This position will assist in developing and overseeing new services for the Children’s waiver, including policies, procedures, and training, and providing oversight and monitoring of the new services and assist federal reporting. Duties may include but are not limited to development of policies and procedures, research and interpretation of regulations and other federal and state requirements, surveillance, quality and performance management, analysis of data and preparation of reports, speaking at both internal and external meetings, educational sessions and webinars, providing technical assistance to providers, and preparing correspondence and other written materials. The incumbent will provide clinical expertise and technical assistance for service requests and modifications to support identified medical needs. Travel will be required, including representing the Department at local and regional meetings and participation in on-site visits of Health Home and HCBS programs. The incumbent may also provide direct supervision of staff.

*Posted salary range includes the statutory hiring rate at step 4and job rate for PS&T Grade 22 ($85,272 - $95,392) and an additional Geographic Pay Differential of $20,000
