Minimum Qualifications MINIMUM QUALIFICATIONS:

1.Must currently be a permanent LMSW 1;

OR

2.Must be active and reachable on the Civil Service eligible list for LMSW 1;

OR

3.Eligible for 70.1 transfer by way of current permanent status in a title deemed comparable to the LMSW 1 by the Department of Civil Service (Must also be within 2 salary grades of the targeted title).

4.Qualified for provisional appointment.

MINIMUM QUALIFICATIONS FOR PROVISIONAL APPOINTMENT:
Provisional Qualifications: Candidates must be licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) AND have one year of post NYS licensure professional social work experience;**

Or Candidates must be licensed and currently registered by New York State as a Licensed Clinical Social Worker (LCSW).
Registration and licensure as a Licensed Mental Health Counselor are not considered qualifying for these examinations.

** Professional social work experience involves activities such as screening, assessing and diagnosing individuals' and families' psychosocial needs, strengths and limitations; participating in the development of treatment plans and discharge plans; counseling and educating individuals, groups and families to promote mental, emotional, physical and social functioning; helping access needed services; providing client/patient advocacy and crisis intervention; and developing, coordinating and evaluating services and support systems. Experience limited to social work research or social work administration is NOT considered qualifying.

Duties Description POSITION DESCRIPTION:

The LMSW 1 will support the functions and processes related to the Waiver team for HCBS Waiver enrollment and service authorization.

PRIMARY DUTIES AND RESPONSIBILITIES:

•Assist Individuals with developmental disabilities and their families in accessing OPWDD supports and services.
•Present Service Amendment Request Forms for authorization at weekly Quality Review Team meetings
•Complete HCBS Waiver enrollment packets for Individuals seeking HCBS Waiver services
•Complete Notice of Decisions for Waiver enrollment and for all service authorizations
•Provide technical assistance to Care Managers on Waiver processes and forms
•Process DDP1 forms in Choices for program enrollments as needed
•Communicate current Waiver processes to Individuals and Families
•Assist in following up on stakeholder concerns related to Waiver processes and report to Regional Field Office Administrators
•Attend Waiver training updates, local, regional, and statewide as needed
•Complete special projects as assigned. These could include a variety of tasks such as assistance on Regional Office policy initiatives and serving as a participant for a workgroup on new initiatives as they arise
•Other duties as assigned
•Travel may be required for meetings and presentations

•Knowledge of voluntary provider agency operations, TABS (Tracking and Billing System), CHOICES, Excel, SharePoint, and other computer skills are valuable assets for this position.
