Minimum Qualifications MINIMUM QUALIFICATIONS:
Permanent Qualifications: Candidates must currently be a permanent LMSW 1 OR must be active and reachable on the current Civil Service eligible list for LMSW 1.

Transfer Qualifications: Candidates must be eligible for 70.1 transfer by way of current permanent status in a title deemed comparable to the LMSW 1 by the Department of Civil Service (Must also be within 2 salary grades of the targeted title).

Non-Competitive Qualifications: For permanent, non-competitive appointment under the Civil Service Hiring for Emergency Limited Placement (HELP) Program, candidate must be
Licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).

Registration and licensure as a Licensed Mental Health Counselor is not considered qualifying.

Duties Description POSITION DESCRIPTION/PRIMARY DUTIES AND RESPONSIBILITIES:

The Licensed Master Social Worker 1 (LMSW 1) would function in the role of Placement Coordinator for Capital District DDSO. In this position, the incumbent is responsible for:
• Managing the community residential, respite and day habilitation opportunities for 75 IRAs, 6 Day Habs, and respite for Family Care in the Capital District DDSO community.
• Working collaboratively with referring entities to gather necessary referral information. Prioritizing highest need individuals for placement.
• Participating in placement meetings and sharing information with the Capital District teams.
• Identifying residential placements based on the needs and choices of individuals and ensuring a good fit between the individual’s needs/choices and the home/day hab/respite (ie: housemates, physical needs such as barrier free, geographic location, proximity to family/supports, etc).
• Identifying individuals who are able to live in a less restrictive setting and assisting with placements.
• Maintaining a tracking system of all referrals and State Operations vacancies.
• Assist with Central Office initiatives that increase flow of individuals through the system.
• Attending meetings as assigned.
• Other duties as assigned.
