Minimum Qualifications MINIMUM QUALIFICATIONS:

NON-COMPETITIVE QUALIFICATIONS: A Bachelor’s or higher-level degree in a human services field*

*For the purpose of these qualifications, a degree in a human service field includes the professional disciplines of audiology, community mental health, nursing, nutrition, occupational therapy, physical therapy, psychology, rehabilitation counseling, vocational counseling, social work, speech/language pathology and therapeutic recreation (including concentrations in art, creative art, dance, drama, music, or physical education.) Also included are related academic disciplines associated with the study of human behavior (e.g., gerontology, human services, sociology, speech communication); or human skills development (e.g., counseling/guidance, education, human development.) Examples of non-qualifying degrees include but are not limited to: criminal justice, political science, liberal arts, and business.

COMPETITIVE QUALIFICATIONS:
Must be in title or on the Civil Service eligible list for this location or be eligible for transfer under Section 70.1 in a designated title determined by the New York State Department of Civil Service.

PREFERRED QUALIFICATIONS:
Qualified Intellectual Disabilities Professional (QIDP) status. Preferred qualifications are at least one year’s experience providing services to persons with developmental disabilities AND either; a master’s degree in a field of psychology, a bachelor’s degree in a human services discipline, a psychology license currently registered with the NYS Education Department, a nursing (or licensed practical nursing) license currently registered with the NYS Education Department, or a master’s degree in social work with current certification and registration with the NYS Education Department as a social worker

Duties Description POSITION DESCRIPTION:
Reports to Northern Erie Team TTL and serves as QIDP (Qualified Intellectual Disability Professional) for the Northern Erie Team.

PRIMARY DUTIES AND RESPONSIBILITIES:

• Develops and updates Individuals Safeguard Summary Plans (SSP).
• Develops and updates residential Staff Action Plans (SAP) with the individual, family/advocate, Program Planning Team, Care Coordinator and service providers.
• Provide weekly/monthly review of service notes documentation for Medicaid Compliance Billing
• Participate in the development and review of Support and Risk Accountability Plans (SRA). Present at SRA meetings.
• Train residential staff on the implementation of Staff Action Plan outcomes and documentation of individual’s response to services.
• Monitor the implementation of Safeguard Summary Plan (SSP), Staff Action Plan (SAP) and Support and Risk Accountability Plan (SRA).
• Review all documentation, assess status and progress of the Staff Action Plan (SAP), Support and Risk Accountability Plan (SRA) and Safeguard Summary Plan (SSP). Revise as needed to assure that plans continue to reflect the individual’s abilities and need for protective oversight and supports. Review billing documentation monthly.
• Submit and Present information for the Behavioral Intervention Rights Committee (BIRC).
• Submit Clinic Referrals; Disseminate copies of completed clinic reports.
• Participate in the Life Plan review meetings, House and Team Meetings, Special Meetings and take minutes.
• Participate in Programmatic Self-Assessment and Regulatory Plans of Corrective Action (POCA) development, implementation and monitoring as relevant to the QIDP role.
• Participate with other team members in a variety of individual, team and continuous improvement meetings and forums as scheduled to promote comprehensive and individualized services.
• Participate in Adjunct Tasks, as assigned by TTL
• Must have the ability to adjust work hours to meet operational/supervisory needs including but not limited to Life Plan Meetings, trainings, and off shift visits to the houses on the team.
