Minimum Qualifications MINIMUM QUALIFICATION:

Permanent service as a Dietitian 2 in a New York State agency eligible for transfer

OR

Current registration with the Commission on Dietetic Registration (CDR) as a Registered Dietitian (RD).

Or Your New York State Education Department (NYSED) certification in dietetics-nutrition

AND have completed a minimum of an additional 400 hours* of supervised dietetics practice under the supervision of a RD.

You must indicate your NYSED certification in dietetics-nutrition number on your application.
* If you are qualifying under #2, the additional 400 hours required is in addition to the 800 hours of supervised dietetics practice under the supervision of an RD required for NYSED certification. On your application you must provide the name of the institution where you obtained the additional hours; the name of your supervisor; and your supervisor's qualifications.


Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:

• Monitor the Nutritional need of Individuals assigned – Develop seasonal menus tailored to the individual’s needs. Develop weekly food shopping list consistent with the menus. Help implement the concept of access to food/ individual first.
• Complete an EHR based Nutritional Assessment – including initial, quarterly etc. via the nutritional care process (NCP) for each individual based on his/her nutritional care needs, ensuring all components of the E.H.R assessment are completed.
• Do interim notes as needed.
• Attend all the meetings including Initial, quarterly, semi annuals, core etc. as needed.
• Ensure monthly weights, lab reviews as available and needed, mealtime guidelines update monthly or as needed.
• Develop training plans for employees on all three shifts in assigned areas based upon individuals' specific nutritional care needs.
• Conduct daily meal rounds to ensure acceptance and tolerance of planned meals.
• Ensure completion of annual trainings for self (e trainings)
• Provide trainings to individuals - including meal planning, shopping, cooking, sanitation to facilitate / promote independent living upon discharge. Encourage participation/ involvement in shopping, cooking, cleaning etc.
• Provide training to staff – shopping, meal planning, textures, cooking, proper food safety and sanitation and other topics as needed.
• Provide nutrition care instructions to individuals and their families.
• Conduct monthly sanitation inspections of kitchens and food safety update.
• Supervise DSAs and FSWs on food and food safety related issues and communicate with NSA and TTL as needed
