Duties Description As a Licensed Master Social Worker 1 (LMSW1) assigned to the Social Services Department at the New York State Veterans Home at Oxford, the incumbent is responsible for, but not limited to, the following:
• MDS assessments, interviews, documentation, data collection and entry into EMR system.
• Preparing new care plans and updating existing care plans
• Documentation in the EMR
• Facilitate and participate in family meetings/Care Plan meetings
• Participate in IDT meetings with Psychiatrist, Medical, Nursing, administration, rehabilitation and admissions
• Serve as family liaison and support resident/family advocacy through regular and as needed contact and intervention
• Case management
• Assist residents and families with admission to units and room moves when indicated
• Coordinate and oversee discharge planning for assigned residents
• Initiate/facilitate end of life discussions and complete advanced care planning, such as completing eMOLSTs with appropriate documentation and care plan development and maintenance
• Complete Trauma Screens and initiate/maintain timely and accurate care plans for such
• Complete Suicide Assessments and initiate/maintain timely and accurate care plans for such
• Assess residents for depression and work with interdisciplinary team to implement appropriate treatment plans
• Conduct interviews with residents and families/health care agents to complete Psychosocial Histories and document pertinent information
• Assess need for Psychotherapy with appropriate referrals and provision of supportive counseling as indicated
• Provide support for Loss, Bereavement, and Grief
• Take an active role in Resident Council with follow up and / or referrals as warranted and requested
• Facilitate Family Council
• Provide Crisis Intervention as per assessed or demonstrated need
• Actively participate in the Medication Management Team with follow up communication and monitoring for any changes as warranted / requested
• Provide Dementia Training to interdisciplinary team on an ongoing basis
• Participate in RAPID meetings to review and assess all resident incidences, such as falls or skin tears and assist interdisciplinary team to develop and implement appropriate care planning
• Participate in Environmental Rounds and work with residents/families as needed to maintain a safe, healthy, and regulatory compliant living space
• Maintain HIPAA Compliance and assist with educating residents, families and staff as needed.
• Provide Transition Support for residents/families
• Complete and document PASRR Evaluations as required
• Assist residents and families with Financial and VA Benefit Coordination
• Participate in Infection Control and Prevention and provide education and information to residents, families, and visitors regarding mitigation of risk and regulatory requirements as needed
• Ensure resident rights are respected and provide education and advocacy as indicated
• Assist residents, families, and visitors to maintain communication when needed
Minimum Qualifications Reassignment: Current New York State Department of Health Veterans Home at Oxford employee with permanent or contingent permanent status as a Licensed Master Social Worker (SG-20).
Transfers: Current New York State employee with one or more years of permanent, contingent-permanent, or 55 b/c service in a position allocated to Grade 18 or above, and that is eligible for transfer under Section 70.1 of the Civil Service Law to Licensed Master Social Worker.
Open Competitive (NY HELPS: license and current registration as a Licensed Master Social Worker in New York State.
Additional Comments Licensed Master Social Workers serves as a member of interdisciplinary treatment teams applying professional social work theory, principles, and methods to assess, evaluate, formulate and implement a plan of action based on individual needs and strengths and intervene to address mental, social, emotional, behavioral, developmental and addictive disorders, conditions and disabilities, and psychosocial aspects of illness and injury.

There is a Shift Differential Pay Increase of 7% for the evening shift 7% and 10% for the night shift.
This position is part of the NYS HELPS program. No examination is required.
