Duties Description The incumbent will be responsible for conducting surveillance activities including onsite surveys and complaint investigations of Adult Care Facilities (ACF) either independently or as part of an interdisciplinary survey team by observing and evaluating resident care against established standards and for evaluating ACF compliance with Federal, State and Department regulations and policies. They will be the surveyor assigned to incoming complaint investigations as well as incident reports, deaths, and significant events. They will be responsible for completing document review and reports accordingly while reducing the backlog. Additionally, this incumbent will review resident termination notices, plans of correction, waivers and equivalences, and facility personnel applications. Additionally, the incumbent will evaluate available data to inform policy, follow-through on such policy, and maintain appropriate documentation to support policy development; generate citations where appropriate and follow-through accordingly. Depending on programmatic need, the incumbent will perform other duties as assigned to support the team, including review for regulatory compliance and quality assurance facility-developed information including policies and procedures, residency agreements, and plans of correction, and prepare proper communication to such facilities outlining a determination on the material submitted.

Minimum Qualifications Permanent Transfer:

Current New York State employee with one year or more of permanent or contingent permanent, competitive service in a Health Systems Specialist 1 (G18) position OR a NYS employee in a position G16 or higher and eligible for transfer under Section 70.1 or Section 70.4 of the Civil Service Law.

Non-Competitive via NY HELPS:

Six years of experience performing or implementing medical or programmatic surveillance, audit, compliance, quality assurance, or regulatory activities for patients or a health care delivery system for a health care facility or health related organization.

Substitution: An associate degree may substitute for two years of experience; a bachelor’s degree may substitute for four years of experience; a master’s degree may substitute for five years of experience; and a doctorate may substitute for six years of experience.

Preferred Qualifications: Strong organization skills and proficiency with the Microsoft Office product suite. A demonstrated ability to interpret policies and analyze workflow. Dedication to a team structure, emotional intelligence to support growth, and a commitment to effectuating constant quality improvement. A strong desire to learn both independently and as part of a multidisciplinary team and apply knowledge to sustainable improvements. Federal Surveyor Minimum Qualifications Test (SMQT) Certification is preferred.

Additional Comments This position requires travel to areas often underserved by public transit. The successful incumbent must be flexible and available to work varied hours, which may include weekends and holidays, to respond to emergencies and/or provide guidance relative to off-hours investigations, as needed. Work hours to be determined at time of hire. Federal Surveyor Minimum Qualifications Test Certification is required within one year of the date of employment. *Specific work hours are TBD within standard department hours.

Posted salary range may include an additional $4,000 location pay.

This position is part of the NYS HELPS program. No examination is required.

This position may require travel throughout New York State including overnights to areas not served by public transportation.
