Duties Description The Office of Mental Health (OMH) is seeking qualified candidates to fill a temporary Mental Health Program Specialist 2, G-23 position in the Division of Community Program Policy & Management, Adult Community Care Group. The Mental Health Program Specialist 2 will assist in the effective implementation of the Post-Settlement and Future State Adult Home Settlement activities to ensure all members have the opportunity to continue to live in the community with needed supports, and conduct post-settlement work.

Specific duties include, but are not limited to, the following:
• Develop policies and provide technical assistance to housing and community providers in providing needed wrap-around supports to ensure community tenure and support individuals in housing as they age.
• Provide ongoing monitoring in the community housing setting and ensure that the goals of the Adult Home Settlement are maintained.
• Coordinate and complete chart reviews and in-person wellness visits for transitioned class members, visiting members in their Supported Housing units, with settlement providers, including AH+ CMs, Peer Bridgers, Housing Staff, and MLTC staff, as needed.
• Conduct on-site visits to various providers.
• Maintain documentation for all assigned cases, wellness visits, chart reviews, database entries, etc.
• Serve as the Escalation Lead for remaining and future “Yes” members.
o Escalation management is a process put in place to deal with incidents and problem management within a system, ensuring problems get resolved and issues are addressed at the right level to expedite cases that require move dates, monitor cases with move dates to keep moves on track, and assist with barriers impacting moves to minimize delays.
• Provide technical assistance to provider team members to ensure transitions to the community setting are implemented in a safe manner.
• Serve as the Transition Lead for cases of concern and incidents, evaluating critical occurrences, meeting with members as needed, and collaborating with settlement providers, to mitigate risks.
• Troubleshoot case-specific problems for class members in the community which may become a reportable incident and to ensure incidents are reported, reviewed and addressed.
• Maintain working relationships with administrators and/or leadership at DOH-licensed Impacted Adult Homes for the purposes of securing documentation (i.e., face sheets), negotiate private meeting spaces, coordinating discharge plans for “Yes” class members, and future SMI in-reach projects.
• Participate in in-person and virtual interdisciplinary team meetings with class members and/or settlement providers, including AH+ CMs, Peer Bridgers, Housing Staff, and MLTC staff.
• Participate in interagency provider conference calls related to status updates on a particular class member’s progress.
o Develop transition planning and evaluate post transition issues needing resolution.
• Communicate weekly with OMH and DOH staff, other agencies and providers to track progress made.
• Facilitate and/or participate in team meetings, pre-transition, and post-transition meetings for “Yes” class members and transitioned members, with settlement providers, including AH+ CMs, Peer Bridgers, Housing Staff, and MLTC staff.
• Make recommendations for next steps and action items to the Mental Health Program Specialist 3 Clinical based upon data and discussion with providers.
• Report back on additional analysis required as the result of meetings and recommend action.
• Assist leadership with recommendations to inform changes to improve the experience of class members served under the Settlement and document compliance with Settlement requirements.
• Look for patterns and trends that indicate broader issues requiring further investigation, such as a problem with a particular provider or Adult Home, to prevent new motions that could restore Action.
• Other duties as assigned.

Minimum Qualifications Non-Competitive Minimum Qualifications Under the NY HELPS Program:

A bachelor's degree in biology and all subspecialties of biology relating to human biology, child and family studies, counseling and community psychology, creative arts therapy, criminal justice, health administration, hospital administration, marriage and family therapy, mental health counseling, nursing, psychoanalysis, psychology, public administration, public health, public policy, recreation therapy, rehabilitation counseling, social work, sociology, or special education and three years of professional clinical or administrative experience in a federal or state operated, licensed, certified, or funded mental health program.

A master's degree in biology and all subspecialties of biology relating to human biology, child and family studies, counseling and community psychology, creative arts therapy, criminal justice, health administration, hospital administration, marriage and family therapy, mental health counseling, nursing, psychoanalysis, psychology, public administration, public health, public policy, recreation therapy, rehabilitation counseling, social work, sociology, or special education can substitute for one year of specialized experience.

Transfer Opportunity:
Candidates must have permanent service as a Mental Health Program Specialist 2, Grade 23, and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1 of the Civil Service Law by having at least one year of permanent service in a qualifying title within two salary grades of the Grade 23 level.

Additional Comments Candidates must note this position is being filled on a temporary and contingent basis, should the incumbent return to their former position, employment in this temporary position will be terminated.

For the duration of the NY HELPS Program, this title maybe filled via a non-competitive appointment, which means no examination is required but all candidates must meet the minimum qualifications of the title for which they apply. At this time, agencies may recruit and hire employees by making temporary appointments.

At a future date (within one year of permanent appointment), it is expected employees hired under NY HELPS will have their non-competitive employment status converted to competitive status, without having to compete in an examination. Employees will then be afforded with all of the same rights and privileges of competitive class employees of New York State. While serving permanently in a NY HELPS title, employees may take part in any promotion examination for which they are qualified.
