Duties Description The Physical Therapist will plan and conduct medically prescribed treatment for patients with physical disabilities resulting from disease or injury utilizing all physical therapy methods. The incumbent would perform the duties of a first-level professional clinician under the supervision of a Senior Physical Therapist. The Physical Therapist will plan, develop, organize, evaluate, and direct physical therapy programs/services for the residents that have specific physical incapacities, in accordance with current rules, regulations, and guidelines that govern long-term care facilities; evaluate residents’ physical condition and symptoms and review medical records; complete annual and condition change physical therapy screening; monitor progress toward goals and adapt treatment plans accordingly; prepare and maintain treatment records, resident files, and progress notes as appropriate. The Physical Therapist will maintain communication & positive relationships with the residents, and the residents’ families and/or health care agents; participate in discharge planning, recommend and coordinate assignment of any mobility equipment needs. The Physical Therapist will also be responsible for overseeing the supervision and training of the Physical Therapy Assistants, Physical Therapy students, and lower direct care staff; will conduct new employee orientation training and ongoing training regarding Safe Patient Handling practices; attend and participate in workshops, seminars, etc., to keep abreast of current changes in the health care field.
Minimum Qualifications Current New York State employee with one or more years of permanent, contingent-permanent, or 55 b/c service
in a position allocated to Grade 18 or above, and that is eligible for transfer under Section 70.1 of the Civil Service Law to Physical Therapist.
Open Competitive HELPS qualifications: If less than three interested and qualified New York State Department of Health employees serving in the direct-line, lower-level title(s) within the agency apply to this opportunity, the agency may make a non-competitive appointment under the NY HELPS program: Possession of a license and current registration as a physical therapist in New York State.

The posted salary range ($80,193 - $100,121) includes a geographic pay differential of $6,000.
