Duties Description Kingsboro Psychiatric Center (KPC) is recruiting for Social Work Assistant 2 to function as a Risk Management Specialist and will play a critical role in Risk Management functions within the facility.
Responsibilities include investigating significant incidents, addressing patient and family complaints, collecting and analyzing data, and ensuring regulatory compliance. The ideal candidate will have excellent communication and organizational skills, with a proactive approach to promoting patient safety and continuous improvement.
Duties include:
• Conduct and oversee all aspects of investigations as per Office of Mental Health (OMH) policies and procedures, including but not limited to:
o Interview staff and gather relevant documentation.
o Coordinate Justice Center interviews with facility leadership for patients and staff.
o Complete comprehensive reports on significant incident investigations within 30 days.
o Identify incident causes, patterns, and trends to inform risk reduction strategies.
• Conduct risk management orientation and related training sessions for staff.
• Participate in environmental rounds to identify and mitigate potential safety risks.
• Review incident and restraint/seclusion forms, ensuring accurate entry into the NIMRS database.
• Serve as the liaison between the facility, OMH field office, Central Office, and the NYS Justice Center.
• On-Call Availability: Be available during off-shifts and weekends to address reportable incidents as needed.
Minimum Qualifications Candidates from outside or within State Service can be considered for hire under the Hiring for Emergency Limited Placement – Statewide (NY HELPS) program if they have a bachelor’s degree in a human service field and 2 year of social work experience.
o Substitution: Master’s degree in social work can substitute for 1 year of specialized experience.

Candidates within State Service can be considered for a competitive class appointment if they are reachable on the current Social Work Assistant 3 eligible list.
OR
Candidates from within State Service can be considered transfer to a competitive class position if they have one year of permanent competitive or 55-b/55-c service in a title eligible for transfer under Section 70.1 of the Civil Service Law.

In accordance with Section 52.7 of the Civil Service Law, this notice advises all employees of the Office of Mental Health of our intention to conduct a non-competitive promotion examination for the position of Social Work Assistant 2 at Kingsboro Psychiatric Center. Permanent appointment may be made by non-competitive promotion of an applicant who meets the position’s minimum qualifications if all requirements of non-competitive promotion are met.
The promotion field for the Social Work Assistant 2 position consists of candidates who have 1 year of permanent service as a Social Work Assistant 1.

Additional Comments:
This position is eligible for a Downstate Adjustment of $4,000.
