Duties Description Reporting to the Director of Rehabilitation Hospital (M-8), the Rehabilitation Hospital Utilization Manager (M-2) will be the Licensed Nursing Home Administrator of the Skilled Nursing Facility (SNF) and Transitional Care Unit (TCU); and will function as the administrator-of-record for the facility and coordinate federal and state regularly survey processes to ensure continued facility accreditation. The Rehabilitation Hospital Utilization Manager will be responsible for operational responsibility for all aspects of the SNF and TCU program development and implementation, legislative and regulatory compliance, policy development, case mix and census management, capital and operational expenditures, operation of physical plants, and strategic plans. The role will have operational oversight in a matrixed leadership organizational structure, in conjunction with nursing and therapy leadership, of SNF and TCU service lines. The role provides leadership, guidance, and oversight to subordinate staff, and intervene and resolve problems as necessary; identifies training needs and review performance ratings of all personnel.
The Rehabilitation Hospital Utilization Manager will provide comprehensive oversight of staff assignments, ensuring strict adherence to compliance and policies across the Skilled Nursing Facility (SNF) and Transitional Care Unit (TCU) service lines. This role entails managing interdisciplinary collaboration, including Nursing, Therapy Services, Information Technology, Nutritional Services, Facility Management, Admissions, Case Management, Utilization Management, and Patient Financial departments. To promote a high standard of care, the incumbent will analyze departmental program data to implement quality improvement activities and meet regularly with department heads to implement updated policies and procedures.
Beyond operational management, the Utilization Manager is directly responsible for resident advocacy and satisfaction, including investigating resident complaints, resolving grievances, and ensuring that resident rights policies are strictly maintained. They will oversee the administration and coordination of all departments to ensure the delivery of high-quality care, acting as a crucial link between departmental processes and resident outcomes within the rehabilitation hospital setting.
In addition to advocating for the Hospital’s position, the incumbent will represent the institution at meetings with the Department of Health, control agencies, and the Board of Visitors regarding proposed budgets, expenditure plans, and operational strategies. This role requires close collaboration with department heads to develop comprehensive budgets while liaising with Health Facilities Management to secure necessary approvals. Furthermore, the Rehabilitation Hospital Utilization Manager holds the authority to approve the purchasing of all equipment, supplies, and foodstuff, and to authorize expenditures for facility maintenance and construction projects.
Minimum Qualifications Transfers: Current New York State employee with one or more years of permanent, contingent-permanent, or 55 b/c service in a position allocated to Grade 25 or above, and that is eligible for transfer under Section 52.6 of the Civil Service Law to Rehabilitation Hospital Utilization Manager.
Open Competitive HELPS qualifications:
A master’s degree in a health care field or public health administration, and three years of experience managing departments involving professional, medical, or other direct patient care services; or a bachelor’s degree in a health care field or public health administration, and five years of experience managing departments involving professional, medical, or other direct patient care services.
Preferred Qualifications: The preferred candidate will have possession of a New York State nursing home administrator license.
A clinical degree in nursing, allied health, or related healthcare field is preferred (Master’s degree preferred).
The ideal candidate will possess demonstrated knowledge of CMS regulations, NYS DOH requirements, and survey readiness processes. Proven success leading facilities through NYS DOH surveys with minimal deficiencies. Strong experience in quality improvement (QAPI), infection prevention oversight, and resident safety initiatives.

The posted salary range ($107,870 to $135,298) includes a downstate adjustment of $4000.
