Minimum Qualifications Permanent Qualifications: Candidates must possess or be eligible to obtain a license to practice as a Licensed Practical Nurse in New York State.
Transfer Qualifications: Candidates must currently be a permanent Licensed Practical Nurse.

Duties Description POSITION DESCRIPTION:
The LPN is needed to provide care within the scope of practice for an LPN. Under the supervision of a Nurse 2 they will provide care to individuals attending Ulster Day Program. This position is included in the staffing minimum on the shift and performs all Direct Care duties, as well as LPN duties.
PRIMARY DUTIES AND RESPONSIBILITIES:
Duties include, but are not limited to:
• Ensures that all medical treatments are performed and are implemented as written and data recorded as required.
• Serves as the key person to ensure that communication occurs in an effective manner between the staff at Milton Day Hab, Residential staff and the Registered Nurse.
• Assists Nurse 2 in maintaining current medical records as required by the Day Hab
• Other duties as assigned
Additional Comments NOTE: You must have a valid driver’s license to apply as travel is a part of the duties involved in this position. This may include a state-owned vehicle, as available, or the employee’s own car. An employee who uses his/her own car will be reimbursed at current negotiated rates.
