Minimum Qualifications Must have a license and current registration to practice pharmacy in New York State. If you are appointed and lose yourlicense or current registration, acton will be taken to remove your position.

Duties Description Provision of pharmaceutical care, including the review, monitoring, dispensing and distribution of inpatient/outpatient meds. Reasonable for providing drug info, conducting med. counseling sessions, supervising pharmacy aides participating in pharmacy and facility wide performance improvement activities. You may be required to provide clinical and distributive services including dur, pharmacotherapy consults, anticoagulation management, health and wellness program participation, dispensing and other clinical distributive services assigned. Actively manage drug therapy buy identifying preventing resolving medication-related problems either directly or as part of the treatment team. Collect, analyze and report on med. therapy related data to enhance patient safety and improve patient care and be responsible for maintaining safe, effective efficient operations in the pharmacy department.

Additional Comments This is an hourly (Per Diem) position.
