For a Licensed Master Social Worker 2, candidates must be currently registered by New York State as a Licensed Master Social Worker AND have one year of post-licensure professional social work experience.

Registration and licensure as a Licensed Mental Health Counselor is not considered qualifying for these examinations.
**Professional social work experience involves activities such as screening, assessing and diagnosing individuals’ and families’ psychosocial needs, strengths and limitations; participating in the development of treatment plans and discharge plans; counseling and educating individuals, groups and families to promote mental, emotional, physical and social functioning; helping access needed services and support systems. Experience limited to social work research or social work administration is NOT considered qualifying.

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:

• The LMSW 1 or 2 would meet with individuals referred by DOCCS/OMH prior to release from incarceration in order to; review services, verifying eligibility status and supporting eligibility determination if needed, initiate the elements of the Front Door process; including completing the DDP2 and the information session. In addition, if the individual is interested in Care Management services, the LMSW 1 or 2 will link the person with a voluntary agency for provision of Care Management.
• Participate as a member of the Certified Residential Opportunities Team to advocate for individual’s requesting residential placement. The LMSW 1 or 2 will initiate the residential referral process to the Regional Office’s Certified Residential Opportunities Team gathering all necessary referral packet components. The LMSW 1 or 2 will keep the committee updated on the client’s residential/legal situation.
• For individuals receiving treatment in an ITO, the LMSW 1 or 2 will participate in quarterly treatment plan meetings and any other scheduled meeting to stay updated on the individual’s status and plan for discharge. When discharge is discussed, the forensic liaison will work with the Certified Residential Opportunities team to secure residential placement if requested upon discharge.
• Participate in statewide ITO calls.
• The LMSW 1 or 2 will contact families to ask about their willingness to support the individual upon release from incarceration. The Liaison will assist the family and individual during the transition to Care Management services.
• In the absence of a Care Manager, the LMSW 1 or 2 may serve as a source of support to the individual in order to assist the person with transitioning from incarceration to community living.
• The LMSW 1 or 2 will be required to maintain a monthly tracking report showing the status of and activity completed for each individual.
• Assist in obtaining documents as needed for Risk Assessments.
• Attend Local Forensic Advisory Committee Meetings.
• Present individuals to the Statewide Forensic Advisory Committee as needed.
• Present individuals to the Statewide Admissions Committee when court orders for secure placement to OPWDD are made by the judicial system.
• Attendance at meetings, conference calls, video conferences as dictated by the role.
• Other responsibilities as assigned.
