Minimum Qualifications Transfer Opportunities: Any permanent Licensed Master Social Worker 2 OR
Any person eligible for 70.1 transfer.

Minimum Qualifications: Either A. Licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) AND have one year of post NYS licensure professional social work experience; ** OR

B. Licensed and currently registered by New York State as a Licensed Clinical Social Worker (LCSW).

** Professional social work experience involves activities such as screening, assessing and diagnosing individuals’ and families’ psychosocial needs, strengths and limitations; participating in the development of treatment plans and discharge plans; counseling and educating individuals, groups and families to promote mental, emotional, physical and social functioning; helping access needed services; providing client/patient advocacy and crisis intervention; and developing, coordinating and evaluating services and support systems. Experience limited to social work research or social work administration is NOT considered qualifying.

Duties Description Willowbrook Liaison/Fair Hearing and Mediation Coordinator/Special Projects.

1. Serves as the day to day liaison between OPWDD Statewide WB Liaison and the field, including state operated and voluntary agency sector providers.

2. Facilitate meetings at the request of the DDRO concerning issues with class members.

3. Provide technical assistance as needed to voluntary and state operated staff serving Willowbrook class members.

4. Communicate information on related issues to the WB Care Manager. Communicate as needed with local Consumer Advisory Board (CAB) representative, act as a liaison between local CAB rep and providers.

5. Ensure that providers are updating and properly notifying appropriate parties of any incidents or potential issues of concern.

6. Ensure that Care Coordinators follow CAB protocols. Monitor and follow up on medical consent requests. Review notification letters to ensure they are completed accurately and timely.

7. Ensure that quarterly caseload reports are completed timely and correctly from State and Voluntary provider agencies. Ensure compliance. Update all reports as required.

8. Meet with providers on an as needed basis to assist, guide and inform of any changes/issues regarding the role of the care coordinator. This includes providing in-services to providers as needed regarding care coordination protocols.

9. Attend upon request meetings involving class members. Attend video conference trainings provided by the OPWDD Statewide Willowbrook Liaison and ensure attendance and handouts are provided.

10. Assist with the completion of special projects as assigned by administration.
