4. 30 College credits in the areas listed below (*) IN ADDITION TO Associates degree in nursing or completion of a three-year nursing program AND three years of professional clinical nursing experience, including at least one year as described below (**).

(*) Acceptable areas include nursing, health care administration, public administration in a health care area, public health, community mental health, community health counseling, psychology, sociology, biology, social work or human services. The 30 credit may include up to two statistic courses.

(**) One year or experience in providing direct services to Individuals diagnosed with mental illness, mental retardation, developmental disabilities, or alcohol or substance abuse in a community setting. A master’s degree in community mental health may be substituted for this experience only.

Duties Description The primary role of the Community Mental Health Nurse will be to perform PASRR evaluations for the region 4 regional office.

Additional responsibilities include:

• Schedule and complete individualized Level 1 screens Level 2 evaluations as assigned

• Complete required forms and issue response within the specified timeframes

• Draft and send response letters to hospitals and nursing facilities in a timely fashion

• Provide accurate information and assistance to people requesting information about the PASRR process

• Track and provide updates on the status of reviews as requested

• Verify OPWDD eligibility or provide a preliminary review to determine whether an individual may have an intellectual or developmental disability and advise stakeholders on the DDRO eligibility determination process when necessary

• Advise stakeholders on available community living options as well as services and supports to assist individuals to live successfully in the community. Link stakeholders to the appropriate DDRO staff and/or CCO for follow up as appropriate

• Enter data into various computer applications and databases, including TABS

• Work collaboratively with other DDRO staff, CCOs/Care Managers, and other stakeholders

• Perform other related duties as requested by DDRO leadership and supervisor

• This position will require travel in the New York City region as evaluations are performed in nursing homes and hospitals.

Posted Hiring Salary includes the sum of the statutory hiring rate ($59,671) for PS&T Grade 19, Increased Hiring Minimum Salary ($3,651), Geographic Pay Differential ($12,871), and Location Pay ($3,026). Maximum salary includes the sum of Job Rate ($75,785), Geographic Pay Differential ($12,871), and Location Pay ($3,026).
