Minimum Qualifications Permanent Transfer Candidates: Current NYS employee with one year of permanent or contingent-permanent competitive service as a Hospital Nursing Services Consultant, G22, or Community Nursing Services Consultant Home Health Services, G22 and eligible for transfer under 70.1 of the Civil Service Law OR Current NYS employee with one year of permanent or contingent-permanent competitive service in a title G20 or higher and meeting the open competitive minimum qualifications listed below.

Open Competitive Minimum Qualifications: Currently licensed and registered as a professional nurse in NYS; AND a minimum of five years of post-licensure health care, public health, community health, or other nursing experience in an appropriate facility setting as defined below. Three years of this experience must have been providing direct patient care in a licensed health care facility, and two years of this experience must have been in nursing supervision, or working in the capacity of a nurse administrator as defined on exam announcement: https://www.cs.ny.gov/examannouncements/announcements/oc-cr/decentralized/20-969.cfm

Preferred Qualifications: The preferred candidate would have experience in conducting surveys of health care settings and reviewing health and/or behavioral health records and would be familiar with services available under the Medicaid program. The preferred candidate would have experience in problem solving and conflict resolution. The preferred candidate would be able to handle multiple diverse tasks in a busy atmosphere and would have strong organizational skills and attention to detail. The preferred candidate would be able to work independently as well as part of a team in planning and implementing work-related activities. The preferred candidate would be able to speak and write effectively in a professional manner with outside agency staff and members of the public. In addition, the preferred candidate would also have a high level of competence using the following computer programs: Microsoft Outlook, Word, and Excel.

Duties Description Within the Office of Community Transitions, the incumbent of this position will provide support for activities related to the transition of individuals with disabilities from institutional and facility settings to community settings with necessary supports. Specific duties include: visiting facilities to engage with the providers facilitating transitions and resolve concerns impeding transition activities; reviewing records that include protected health information; and assisting with special projects as assigned. Regular travel is required.

Additional Comments Permanent, full-time. This position involves significant travel which may include locations not served by public transportation. Current registration as a Registered Nurse (RN) with the New York State Education Department must be maintained for continued employment in title. Plus additional $3,026 location pay, and $5,945 geographic differential.
