Minimum Qualifications MINIMUM QUALIFICATIONS:

Candidates must be reachable for appointment from the current Civil Service eligible list for the title

OR

Candidates must have permanent competitive status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section (i.e. 70.1, 70.4, 52.6) of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades

OR

In the event that there is not a valid eligible list, candidates who meet the following open competitive qualifications may be qualified for a provisional appointment. To qualify, candidates must have a bachelor`s degree and two years of professional clinical or administrative experience in a federal or state certified DD program agency. One year of this experience must have been in a professional administrative position in which a major duty involved planning, coordinating, and/or evaluating a statewide, regional, or DDSO program for people with developmental disabilities.
Substitution: A master`s degree in a human services specialty or in public administration, business administration or hospital administration may be used to substitute for up to one year of the non-specific experience.

Promotion qualifications, candidates must be a qualified employee of the New York State Office for People With Developmental Disabilities and have had three months of permanent competitive or 55-b/55-c service as: a Developmental Disabilities Program Assistant, Developmental Specialist 1, Developmental Specialist 2, Developmental Assistant 3, Dietitian 1, Dietitian 2, Habilitation Specialist 1 (and parenthetic titles), Habilitation Specialist 2, Nurse 1 (and parenthetic titles), Nurse 2 (and parenthetic titles), Occupational Therapist, Occupational Therapy Assistant 2, Physical Therapist, Physical Therapy Assistant 2, Psychology Assistant 2, Psychology Assistant 3, Recreation Therapist (and parenthetic titles), Recreation Worker (and parenthetic titles), Rehabilitation Assistant 2 (and parenthetic titles), Rehabilitation Counselor 1, Senior Recreation Therapist (and parenthetic titles), Social Work Assistant 2 (and parenthetic titles), Social Work Assistant 3 (and parenthetic titles), Teacher 3 or Teacher 4 (and parenthetic titles). Note: Strong computer skills and experience with incidents are preferred.

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:

• Provide real-time oversight of the critical function of incident management on a statewide basis. Ensure appropriate information is provided for each initial report of serious reportable incidents, allegations of abuse and all deaths. This includes ensuring adequate protective measures for the individuals are taken, that the incidents are classified correctly, and a thorough description of each incident has been received, and that required notifications to entities such as law enforcement are completed as appropriate.

• Work in conjunction with the Office of Investigations and Internal Affairs and the Office of Employee Relations, as well as the Statewide Committee on Incident Review (SCIR) and Local Incident Review Committees, to ensure a strong, coordinated approach is taken in response to any abuse allegations or other serious incidents that may occur within state-operated services.

• Manage the Incident Report and Management Application (IRMA) and review and analyze the data to identify significant trends to ensure they are responded to appropriately to improve the quality of services to individuals served.

• Provide ongoing technical assistance and direction to both State-operated and voluntary provider agencies in the area of incident management.

• Generate Critical and Significant Event reports based on established criteria on an ongoing basis.

• Respond to inquiries relative to incident management as appropriate.

• Develop an understanding of the Part 624 (14 NYCRR Part 624) handbook, commentary and FAQs and provide advice and direction as appropriate.

• Provide DQI staff (Bureau of Program Certification and Continuous Quality Improvement) with pertinent information for monitoring and evaluation and alert Bureau of Program Certification staff to significant untoward events for immediate follow up, as needed.


• Receive corrective action plan requests. Follow up on required information and review responses from providers to corrective action plan requests. Enter appropriate related information into IRMA and/or the VPCR. Complete all follow up as necessary.

• Review of investigations and investigative records. Processing of investigative and related incident records as required. Interaction with oversight agencies and provider agencies relative to investigative records procedures and process and follow up as necessary.

• Complete all other tasks relative to incident management as assigned.
