Minimum Qualifications MINIMUM QUALIFICATIONS:
License AND current registration certificate, issued by the education department, to practice as a Practical Nurse in New York State.
Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
The incumbent will provide 24-hour nursing care to individuals with intellectual and developmental disabilities on the evening shift. The duties will include but not limited to performing specialized nursing care such as peg feeding, medication administration, and dressing. The LPN is responsible for the daily observation and reporting changes in client status to the RN, Physician, and document accurately all incidents and occurrences in a timely fashion.
Prepare quarterly nursing review and attends ISP meetings. Attends to personal care, self-care, and other needs of those individuals who require assistance while always encouraging choice and independence to the most considerable extent possible daily. Treats all individuals with dignity and respect and maintain an individual’s rights based on policy and procedure.
Additional Comments Please note, posted Hiring Salary includes the sum of the statutory hiring rate for ISU Grade 10(38,875), Geographic Pay Differential ($2,000), and Downstate Adjustment ($3,026). Geographic Pay Differential and Downstate Adjustment added to the job rate
($47,709) for the ISU Grade 10.
