Minimum Qualifications **Permanent/Provisional**

Permanent Qualifications: Candidates must currently be a permanent Community Mental Health Nurse OR be reachable on the current Civil Service eligible list.

Transfer Qualifications: Candidates must meet the requirements to transfer under Section 70.1 of the Civil Service Law.

Provisional Qualifications: Candidates must be licensed and currently registered to practice nursing in New York State, or possess a limited permit to practice nursing in New York State. (You must obtain your New York State license within one year of the date of appointment or action will be taken to remove you from the position).
AND EITHER
1. A bachelor’s or higher degree in one of the areas listed below*, AND two years of professional clinical nursing experience, including at least one year as described below** OR
2. 30 college credits in the areas listed below* IN ADDITION TO an associate degree in nursing or completion of a three-year nursing program (attach a copy of your college transcript(s)) AND three years of professional clinical nursing experience, including at least one year as described below**.

* Acceptable areas include nursing, health care administration, public administration in a health care area, public health, community mental health, community mental health counseling, psychology, sociology, biology, social work or human services. The 30 credits may include up to two statistics courses. Courses/degrees in physical education, recreation therapy, English, history, math, anthropology, languages, etc. are NOT acceptable.
** One year of experience in providing direct services to individuals diagnosed with mental illness, mental retardation, developmental disabilities, or alcohol abuse in a COMMUNITY SETTING, e.g., a halfway house, visiting nurse, or a community clinic. SUBSTITUTION: A master’s degree in community mental health may be substituted for this experience only.

Duties Description The Community Mental Health Nurse (CMHN) position will provide administrative oversight to assigned Family Care Homes. They will also work within the framework of a treatment team providing nursing service to individuals diagnosed with developmental disabilities, and their caretakers/family care providers. The CMHN visits the individual’s home to make recommendations for emergency or continuing care, suggest alternative treatment plans, and teach the Family Care Provider (FCP) methods of dealing with emergencies. They also ensure that the health care needs of the individuals are provided and work with appropriated community agencies as necessary and ensure a coordinated team treatment plan.

PRIMARY DUTIES AND RESPONSIBILITIES:
• Monitor the homes to ensure that safety and health issues are immediately addressed and the required follow up is completed
• Ensure that FCP’s are adhering to the Family Care policies relative to health and safety of the individuals while in their Family Care homes
• Ensure that all medical treatments are performed and implemented as written and data is recorded as required
• Provide oversight of individual’s health including participation in the Family Care teams’ coordination of the individual’s health care services
• Communicate and coordinate with other caregivers, including day program providers and health care providers
• Participate in assessing the Family Care provider’s skills and abilities, relative to addressing the needs of the individual
• Provide training and education to both the FCO and the individual regarding health care issues and preventative health strategies
• Participate in the assessment of the home’s physical environment and in the development of recommendations related to home modifications – as appropriate to meet the needs of the individual
• Develops individualized plans of nursing service
• Act as a liaison between the FCP, individuals and other treatment team members

Additional Comments Travel within the five-county catchment area will be required in the performance of the duties of the position. This
may include a state-owned vehicle, as available, or the employee’s own car. An employee who uses his/her own car will
be reimbursed at current negotiated rates.
