Duties Description POSITION DESCRIPTION:
The Developmental Disabilities Program Specialist 1 (DDPS1) will function as the Vacancy Management and Flow Coordinator for Taconic DDSOO, which is comprised of 5 counties (Putnam, Dutchess, Ulster, Columbia, and Greene) serving approximately 400 individuals in a variety of settings (family care, supervised apartments, IRA's, and ICF's), and spanning approximately 3,500 square miles from the Hudson Valley to the Capital District. The incumbent in this position will report to a DDPS4 and will perform duties primarily dedicated to internal movements and reconfigurations, placement of age out and return to district individuals in Taconic's special needs settings, and continuing the agencies efforts to find alternate placement for lesser needs individuals in voluntary provider agencies, family care, and self-direction. This DDPS1 will also function as the Personal Protective Equipment (PPE) Coordinator as well as Electronic Health Record (EHR) Coordinator, and participate in the Medication Administration Taskforce Committee.

PRIMARY DUTIES AND RESPONSIBILITIES:
Primary responsibilities of this position will include, but are not limited to:
• Screen individuals seeking placement within state operated homes and review referral packets
• Planning for and placement of specialized populations, as well as maintaining data collection and spreadsheets to keep track of all movements and reconfigurations
• Complete screening reports to submit as appropriate (i.e., DDRO, Central Office, Administration)
• Maintain a good working knowledge of the layouts, populations, and capacities of sites within the district
• Track all internal and external movements and maintain a database of available vacancies
• Match individuals to vacancies within state operated homes and share referral packets with teams in all five counties
• Attend AROC and developmental center closure meetings
• Oversee coordination of emergency placements throughout the district
• Oversee the district's PPE data entry in the statewide PPE Share Point system to ensure an adequate supply is maintained and provide training to all staff on the use of the system
• Participate in statewide planning and preparedness meetings, as well as local focus groups, to prepare for overall universal precaution needs and to ensure future pandemic or emergency response is well coordinated
• Chairperson for FLOW initiative and Joint Reconfigurations meetings
• Function as placement liaison with DDRO, and collaborate with department heads to strategize optimal supports for placements
• Function as MFP lead for Taconic's three remaining ICF residences
• Supervise an assistant to the PPE Coordinator, as well as oversee several members of the PPE distribution team
• Serve as liaison to Central Office in the areas of PPE, FLOW, and EHR
• Complete special assignments as directed by the Director, Deputy Director, or the DDPSIV

may include a state-owned vehicle, as available, or the employee’s own car. An employee who uses his/her own car will
