Transfer Qualifications: Be eligible for a Section 70.1 transfer into a Habilitation Specialist 2 title

Provisional Qualifications: Candidate must be a qualified employee of the New York State Office for People with Developmental Disabilities and have one year of permanent competitive or 55-b/55-c service as a Habilitation Specialist 1, Habilitation Specialist 1 (Spanish Language), or Habilitation Specialist 1 (Manual Communications).

PREFERRED QUALIFIFCATIONS:
* QIDP (Qualified Intellectual Disorder Professional) designation requires one year's experience working with
people with a developmental disability AND a bachelor’s degree or higher in a human services field (including,
but not limited to sociology, special education, rehabilitation counseling or psychology).

Duties Description POSITION DESCRIPTION:
The Habilitation Specialist 2 (HS2) position is responsible for helping residential participants reach their optimum level of functioning by developing individual habilitation plans, setting goals, and monitoring an individual’s progress in reaching these goals. The HS2 will supervise 2 Habilitation Specialist 1s. The HS2 will be based out Wiltwyck IRA and provide oversight for 13 homes. These locations are subject to change.


PRIMARY DUTIES AND RESPONSIBILITIES:
The Habilitation Specialist 2’s duties will include, but are not limited to:
• Supervise Habilitation Specialists 1s and provide oversight and implementation of habilitation plans
• Develop and oversee residential habilitation plans that address the outcomes as reflected in the ISP as well as the needs and interest of the assigned individuals to provide clear direction to residential staff on methods to assist the individual to acquire, improve, and maintain life skills.
• Develop, approve, and assist in specialized projects for individuals.
• Implement habilitation and program plans, activities, and intervention strategies for individuals.
• Responsible to ensure the completion of monthly notes and send monthly report to Treatment Team Leaders
• Conduct annual service planning training for Habilitation Specialists 1 and other staff. Provide on-going training of residential/habilitation plans.
• Ensure that appropriate documentation occurs for all activities.
• Formulate strategies for provision of services
• Other duties as assigned
